SAT:R (Scar Assessment Tool: Rhinoplasty): A Novel Method for Open Rhinoplasty Scar Assessment

Aims: This study aimed to evaluate and validate the Scar Assessment Tool: Rhinoplasty (SAT:R), a novel tool developed to objectively assess transcolumellar incisional scars following open rhinoplasty. The authors hypothesised that SAT:R would demonstrate satisfactory inter-rater agreement and correlate with existing scar evaluation scales. Methodology: A single-centre observational study was conducted, where two blinded clinicians evaluated clinical photographs of 34 patients post-open rhinoplasty. Initial assessments were made using a visual analogue scale (VAS) ranging from 1 (‘best possible appearance’) to 10 (‘worst possible appearance’). Subsequently, the SAT:R scale was applied, which consists of four dichotomous categories: Colour, Notching, Width and Overall Impression, with scores ranging from 0 to 5 assigned to each category. The total SAT:R score was calculated by summing the scores across the four categories. Inter-rater agreement was measured using the weighted kappa coefficient for both SAT:R and VAS scores. Results: The study found moderate inter-rater agreement for SAT:R, with a correlation coefficient for overall SAT:R scores of 0.637 (p<0.001) and a Pearson correlation of 0.894 (p<0.001). The Width category had the highest inter-rater agreement (correlation coefficient of 0.727, p<0.001). In comparison, the VAS exhibited lower inter-rater agreement, with a correlation coefficient of 0.336 (p<0.001) and a Pearson correlation of 0.703 (p<0.001). Conclusion: The SAT:R demonstrates moderate inter-rater agreement, surpassing that of the VAS. This tool offers a more comprehensive assessment of scar characteristics, thereby enabling nuanced comparisons of surgical outcomes. A broader cohort of evaluators shall be employed for further scar grading, and different methods of incision and closure in rhinoplasty will be evaluated by this novel system.
